If you would like to attend the SHM 2024 Annual Banquet Please complete the following:

If someone invited you to attend, please let us know so we can make sure you are seated together.

Once we receive your information, we will email you an evite. To reserve your seat, click “Yes” on the evite. You will receive a confirmation email that your seat has been reserved.

Note: If you and your spouse share an email, you will receive two evites. BOTH must be clicked to reserve your seats.
